Introduction
Over time, breast shape and position naturally change due to factors such as pregnancy, breastfeeding, weight fluctuations, gravity and ageing. These changes can result in breast sagging (ptosis) and a loss of youthful appearance.
A mastopexy, or breast lift surgery, is designed to lift and reshape the breasts, restoring a firmer, more youthful look without significantly changing breast size. This guide explains what the procedure involves, who it’s suitable for, and what to expect during recovery.
What Is a Mastopexy?
A mastopexy repositions the breast tissue and nipple-areolar complex to a higher, more youthful position. The procedure often includes:
✅ Removing excess skin to tighten the breast envelope
✅ Reshaping the breast tissue to restore aesthetic shape and projection
✅ Repositioning the nipples to an appropriate location (centre of the new breast mounts)
⚠️ Important: Mastopexy does not significantly change breast volume. Patients seeking more fullness may consider a breast lift with augmentation (augmentation-mastopexy) using implants or fat transfer.
Who Is a Good Surgical Candidate for Breast Lift Surgery?
You may be suitable for mastopexy if you:
- Have sagging breasts (with nipples pointing downward or below the breast crease)
- Are happy with your current breast volume but want a better, lifted shape
- Are in good health and at a stable weight
- Have realistic expectations and are not currently pregnant or breastfeeding
✅ Common Candidates:
- Women after pregnancy or breastfeeding
- Women after significant weight loss (but have now achieved the stable ideal weight)
- Women with age-related breast laxity
Mastopexy Techniques and Incisions
Your surgeon will choose the incision pattern based on the degree of lift required:
- Peri-areolar (Donut Lift) – Incision around the nipple only; for mild sagging.
- Circum-vertical (Lollipop Lift) – Incision around the nipple and down to the breast fold; for moderate sagging.
- Wise Pattern / Anchor (Inverted-T Lift) – Incision around the nipple, vertically, and along the breast fold; for significant sagging or skin excess.
Each technique aims to lift and reshape the breast while keeping scars as discreet as possible. The surgeon will also aim to keep the blood supply to the nipple and areola whilst doing the repositioning.
How to Prepare for Surgery
- Choose a Qualified Specialist Plastic Surgeon
Select a surgeon with FRACS (Plastic & Reconstructive Surgery) certification and experience in aesthetic breast surgery. - Pre-Surgery Planning
- Stop smoking and avoid certain medications as instructed
- Maintain a stable weight for optimal results
- Arrange 1–2 weeks off work for recovery
- Discuss Combined Procedures
Some patients choose to combine mastopexy with breast augmentation for added volume or breast reduction for volume reduction.
What to Expect During Recovery
- First Week: Expect swelling, mild bruising, and tightness; pain is typically well-controlled with medication.
- Return to Work: Desk work is usually possible after 7–10 days; avoid heavy lifting for 4–6 weeks.
- Final Results: Shape and scars improve over 3–6 months, with full maturation over 12 months.
A supportive post-surgical bra is usually recommended for the first 6 weeks.
Risks and Safety Considerations
Mastopexy is safe in qualified hands, but potential risks include:
- Changes in nipple sensation (temporary or permanent)
- Scarring or delayed wound healing (more common amongst smokers)
- Asymmetry or need for revision surgery (rare)
Following your surgeon’s pre-op and post-operative instructions minimises these risks and optimises your outcome.
How Long Do Results Last?
A breast lift can rejuvenate breast shape for many years, but it cannot stop the natural effects of gravity, ageing, and weight changes. Wearing supportive bras and maintaining a stable weight helps prolong results.
Is a Mastopexy Right for You?
A breast lift can restore confidence and a more youthful silhouette without changing breast size dramatically. Patients who also desire more fullness may benefit from a combined lift and augmentation for enhanced results.
