Introduction
As we age, the brows can descend, leading to a tired, heavy, or even angry appearance. A brow lift, or brow repositioning surgery, is a procedure that elevates the eyebrows and smooths the forehead to restore a more refreshed expression.
This guide explains what a brow lift involves, who it is suited for, the different techniques available, and what patients can expect during recovery.
What Is a Brow Lift?
A brow lift addresses:
- Low or drooping brows that create heaviness over the upper eyelids
- Horizontal forehead lines
- Frown lines (glabellar creases) between the eyebrows
The goal is to lift and reposition the brow, open the eyes, and improve facial harmony — all while maintaining a natural, non-surprised look.
✅ A brow lift is often performed on its own or combined with blepharoplasty (eyelid surgery) or facelift procedures.
Types of Brow Lift Techniques
- Endoscopic Brow Lift
- Minimally invasive
- Small incisions hidden in the hairline
- Uses a camera to guide repositioning of the brow tissues
- Direct Brow Lift
- Incision placed just above the brows
- Suitable for older patients or those with significant brow ptosis
- Often used in functional cases
- Temporal (Lateral) Brow Lift
- Lifts the outer portion of the brow
- Subtle and natural-looking result
- Can be performed with upper eyelid surgery
- Ponytail Brow Lift
- Similar to Temporal Brow Lift
- But with additional central hairline incisions to introduce internal dissolving cable sutures (to create the effect of a tight ponytail) to elevate the brow position and alter the brow angle / shape
- Reserved for selected patients wanting specific look
- Similar to Temporal Brow Lift
- Pre-Trichophytic (Hairline Lowering) Brow Lift
- Incision made at frontal hairline margin
- Forehead elevated with excess skin excised
- Allow lowering of a high hairline
- Coronal Brow Lift (less common nowadays)
- Incision placed within the hair bearing scalp
- Could lengthen forehead / elevate hairline if required
Your surgeon will recommend the most appropriate technique based on your anatomy, goals, and hairline position.
Who Is a Good Candidate for a Brow Lift?
You may benefit from a brow lift if you:
- Notice heaviness over the eyes or a tired expression
- Have low-set brows or forehead wrinkles
- Want a more open, alert, or youthful appearance
- Are in good health and have realistic expectations
✅ This procedure is suitable for men and women, typically from their 40s onward, depending on genetics and skin laxity.
How to Prepare for Surgery
- Choose a Specialist Plastic Surgeon
Ensure your surgeon holds FRACS (Plastic & Reconstructive Surgery) and has experience in facial aesthetic surgery, including brow and eyelid surgery. - Medical Clearance & Planning
- Avoid smoking and blood thinners
- Arrange for 1–2 weeks off work
- Discuss other facial procedures if combination surgery is appropriate
What to Expect During Recovery
- Swelling & Bruising: Common around the eyes and forehead; subsides within 1–2 weeks
- Dressings & Sutures: May be removed around day 5–7, depending on technique
- Return to Work: Most patients resume office work by 7–10 days
- Final Results: Refined brow shape and improved expression by 6–8 weeks, with subtle changes continuing over months
✅ Scars are usually well-hidden within the hairline or along natural creases, and fade over time.
Risks and Safety Considerations
While brow lift surgery is safe, potential risks include:
- Temporary or permanent numbness along the scalp
- Hair thinning along incisions (usually temporary)
- Asymmetry or overcorrection (minimised with experienced technique)
- Rare complications such as nerve injury or bleeding
Choosing a qualified and experienced surgeon greatly reduces these risks.
Is a Brow Lift Right for You?
A brow lift can make the eyes appear more open and the face more rested, without changing your unique expression. Patients often report looking less tired and more confident, with results that last for many years.
